Hotel Construction Project/6310 Old Station Road, North East, PA/Roof Areas: On or about March 5, 2015 and at times prior there to, guardrail systems were utilized to as a method of fall protection at window openings, wall openings, and pipe chases throughout the multi-story structure. The guardrail systems were not equipped with a mid-rail. The condition exposed employees to fall hazards ranging from 6-feet to 30-feet.
