29 CFR 1910.23(a)(2): Every ladderway floor opening or platform shall be guarded by a standard railing with standard toeboard on all exposed sides (except at entrance to opening), with the passage through the railing either provided with a swinging gate or so offset that a person cannot walk directly into the opening. a. Located on the drain rack in Bay 2, employees removed parts from vehicles where there was an access ladder that did not have a swinging gate or an offset that a person could not walk directly into the opening, thereby exposing the employees to a fall hazard of 5 feet, 8 inches.

29 CFR 1910.23(c)(1): Every open-sided floor or platform 4 feet or more above adjacent floor or ground level shall be guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides except where there is entrance to a ramp, stairway, or fixed ladder. The railing shall be provided with a toeboard wherever, beneath the open sides, a. Located on the working surface of the drain rack in Bay 1 and 2, employees were required to manually open and close metal gates by hand, thereby exposing the employees to a fall hazard of five feet, eight inches. b. Located on the working surface of the drain rack in Bay 2, employees removed parts from vehicles where there was a gap between two sections of railing that had a minimum opening of 25 inches between the vertical posts, thereby exposing the employees to a fall hazard of five feet, eight inches.

The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks shall be in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in Sec. 1910.6. a. Located in the production building, employees stored small propane and MAPP gas cylinders with oxygen cylinders inside of a flammable materials cabinet, thereby exposing the employees to a fire hazard.

29 CFR 1910.253(b)(2)(ii): Inside of buildings, cylinders shall be stored in a well-protected, well-ventilated, dry location, at least 20 (6.1 m) feet from highly combustible materials such as oil or excelsior. Cylinders should be stored in definitely assigned places away from elevators, stairs, or gangways. Assigned storage spaces shall be located where cylinders will not be knocked over or damaged by passing or falling objects, or subject to tampering by unauthorized persons. Cylinders shall not be kept in unventilated enclosures such as lockers and cupboards. a. Located in the rear of the production building, the employer failed to secure an oxygen cylinder in order to prevent the bottle from being knocked over and/or damaged, thereby exposing the employees to a struck-by hazard.
