You must enter information about your business at the top of the OSHA 300 Log, enter a one or two line description for each recordable injury or illness, and summarize this information on the OSHA 300-A at the end of the year. a. On or before July 6, 2017, for the calendar year 2017 OSHA 300 log, the employer had not included all the information in column F as required for the following entries: a. Case no. 50075-2017-004 did not include the object that directly injured the employee. b. Case no. 50075-2017-005 did not include the object that directly injured the employee. c. Case no. 50075-2017-006 did not include the object that directly injured the employee or the identification of which finger suffered the injury. d. Case no. 50075-2017-0010 did not include the object that directly injured the employee or the identification of which finger suffered the injury.

29 CFR 1910.157(e)(3): The employer shall assure that portable fire extinguishers are subjected to an annual maintenance check. Stored pressure extinguishers do not require an internal examination. The employer shall record the annual maintenance date and retain this record for one year after the last entry or the life of the shell, whichever is less. The record shall be available to the Assistant Secretary upon request. a. On or before July 6, 2017, where employees operated the L90G wheel loader, there was no inspection tag on the fire extinguisher. b. On or before July 6, 2017, where employees operated the S-80 JDS-2 forklift, the fire extinguisher inspection expired in May 2017. c. On or before July 6, 2017, where employees operated the S-80 forklift, the fire extinguisher inspection expired in March 2017.
