The certification should identify the machine or equipment on which the energy control procedure was being utilized, the date of the inspection, the employees included in the inspection, and the person performing the inspection. Employees in the dismantling area were exposed to a serious struck by hazard from the material loader that was in operation, without the rubber cover on the accelerator pedal. Including but not limited to, the Volvo wheel loader Model L90F, Pin number VCE0L90FJ00068706, known to the company as 1722.
