29 CFR 1926.501(b)(10):  Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:      a)  Employees engaged in low-slop roofing work were exposed to falls from the roof of a nine story building.      No abatement certification or documentation is required for this item.

(a)  The employer did not verify compliance with paragraph (a) of this section by preparing a written certification record of worker training for those workers engaged in low-slope roofing work.    NOTE:  Written certification shall contain the name or other identity of the employee trained, the date(s) of the training and the signature of the person who conducted the training or signature of the employer.     In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:  a)  Employees performing roofing work from a low slope roof were exposed to the hazard of free falling more than 6 feet  while using personal fall arrest systems that were not rigged to prevent this hazard.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
