29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a. On or about October 20, 2014, the employer failed to ensure an employee troubleshooting the Quintax CNC Router (trim saw 6) were protected against accidental contact with the cutting blade. The machine did not have proper sized safety mats in place. b. On or about October 20, 2014, the employer failed to ensure employees cleaning the Quintax CNC Router (trim saw 6) were protected against accidental contact with cutting blades. The machine did not have proper sized safety mats in place. c. On or about December 20, 2014, the employer failed to ensure employees operating the Union Roller Coaters, series 45 Dope Roller were protected against accidental contact with the rollers. The machine had ineffective machine guarding in place. d. On or about December 20, 2014, the employer failed to ensure employees operating the Automated Hydraulic Press Line, MP 16, were protected against accidental contact with the point of operation. The light curtains were set 35 inches above the ground.
