29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):    At the time of inspection, the employer failed to ensure that employees tasked with removing scrap pieces form the Quintax CNC Router, were adhering to the following steps prior to entering into/near the router and tables:    1)     Preparation for shutdown,  2)     Machine or equipment shutdown,  3)     Machine or equipment isolation,   4)     Lockout or tagout device application,  5)     Disconnecting/Releasing stored energy,  6)     Verification of isolation of energy.       NOTE: It is acceptable to abate this hazard with machine guarding methods.
