On or about 10/21/14 located in Building A on top of the oven, the employer failed to ensure that there were no unprotected floor openings; guardrails were missing leaving a 27 inch opening with a 11.6 foot fall hazard from the lower level.

29 CFR 1910.147(b): Authorized employee. A person who locks out or tags out machines or equipment in order to perform servicing or maintenance on that machine or equipment. On or about 10/21/14 located in building B, the Do-All-Saw and the Hem saw the employer failed to follow procedures that clearly and specifically outline the scope, purpose, authorization, for locking out. The employer was placing locks on the saws and putting the key in their pocket and letting the employees work on the saws. The employer is not the Authorized employee.
