29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:Note:  Toeboards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard.  On or about 10/21/14 located in Building A on top of the oven, the employer failed to ensure that there were no unprotected floor openings; guardrails were missing leaving a 27 inch opening with a 11.6 foot fall hazard from the lower level.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):  A) On or about 10/21/14 located in building A, the Oven electrical panel board conduit box cover was missing with exposed conductors; the employer failed to replace the cover to guard against accidents from employees hitting the conductors while servicing the heater on top of the oven.

29 CFR 1910.147(b): Authorized employee. A person who locks out or tags out machines or equipment in order to perform servicing or maintenance on that machine or equipment.    On or about 10/21/14 located in building B, the Do-All-Saw and the Hem saw the employer failed to follow procedures that clearly and specifically outline the scope, purpose, authorization, for locking out. The employer was placing locks on the saws and putting the key in their pocket and letting the employees work on the saws. The employer is not the Authorized employee.
