29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. a) Site, Bridge 2 - On or about 10/22/14 and at times prior thereto employees walked and worked around missing steel panels in the area of the bridge columns. The holes in the bridge floor were left unprotected, and employees were exposed to falls from an approximate elevation of 40.0 ft. to ground level. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface having an unprotected side or edge which was six or more feet (1.8m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems: a) Site, Bridge 2 - On or about 10/22/14 and at times prior thereto employees walked and worked around the S-SE corner of the bridge. The border was not guarded and employees were exposed to falls from an approximate elevation of 40.0 ft. to ground level. b) Site - On or about 10/22/14 and at times prior thereto employees walked and worked around the space between Bridge 1 and Bridge 2, on the S end of the bridges. The border was not guarded and employees were exposed to falls from an approximate elevation of 30.0 ft. to ground level. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
