29 CFR 1926.100(a): 1926.100(a):  At this job site, employees exposed to possible head injury from falling or flying objects or from impact with equipment, hand tools and overhead blocklaying operations, were not wearing hard hats.  (a) Interior Wall - On or about October 23, 2014 an employee was not wearing a hard hat while working under scaffolding. The employee was exposed to being struck-by a steel base plate being used above.

29 CFR 1926.451(b)(1): 29 CFR 1926.450(b)(1):  Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports:   (a) Jobsite - On or about October 23, 2014 employees were working on scaffolding that was not fully planked.   - Interior Wall. The employee was exposed to a fall height of 10 feet. - Exterior Wall. The employees were exposed to a fall height of 14 feet.

29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used.   (a) Jobsite - On or about October 23, 2014 employees used the supports of masonry scaffolds to access the top level of each scaffolding.The scaffolds did not have a ladder attached to access the work area.        -  Interior Wall Scaffold. The employee was exposed to a fall height of 10 feet.     -  Exterior Wall Scaffold. The employees were exposed to a fall height of 14 feet.
