29 CFR 1910.304(f)(1)(iv): Overcurrent devices for circuits rated 600 volts, nominal, or less, were not readily accessible to each employee or authorized building management personnel.  Bay 3 and Bay 8; on or about 21 October 2014 and times prior thereto; the employer did not ensure that the electrical panels were readily accessible in the event of an electrical emergency. Two instances were noted:      a. Bay 3, Panel labeled D, had a floor jack stored in front of the panel.         b. Bay 8, Panel labeled A1, had a push broom stored in front of the panel.  This condition exposed employees to an electrical shock hazard.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure.  Bay 3 and 4; on or about 21 October 2014 and times prior thereto; two instances were noted where flexible extension cords were substituted for fixed wiring:      a. Bay 3; a 25 foot orange extension cord servicing a John Bean tire changing machine, SA 176222, Model EEWH306A1PH, Serial number not legible, was permanently mounted to the wall and attached to the pneumatic line.           b. Bay 4; a 15 foot black extension cord servicing a John Bean tire changing machine, SA 176222, Model EEWH306A1PH, Serial Number 06106025063006994 was permanently mounted to the floor and covered with angle iron.  This condition exposed employees to an electrical shock hazard.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws.  Bay 4; on or about 21 October 2014 and times prior thereto; strain relief was not provided for a 15 foot orange flexible extension cord, connected to John Bean tire changing machine, SA 176222, Model EEWH306A1PH, Serial Number 06106025063006994. This condition exposed employees to electrical shock hazards.
