29 CFR 1910.37(a)(4): Safeguard(s) designed to protect employees during an emergency (e.g., sprinkler systems, alarm systems, fire doors, exit lighting), were not in proper working order at all times. Storage room; on or about 21 October 2014 and times prior thereto; two instances were noted where the emergency lights were inoperative: a. In the general storage area; the emergency exit lights were inoperative b. In the beverage storage area; the emergency exit lights were inoperative. This condition exposed employees to delays in exiting the building and smoke inhalation, fire, and tripping hazards.

29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries. Day room, adjacent to dart board; on or about 21 October 2014 and times prior thereto; an Amerex ABC portable fire was not mounted. This condition exposed employees to a fire hazard.
