29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries. Shop bay, adjacent to electrical panel labeled A1; on or about 21 October 2014 and times prior thereto; an Amerex ABC portable fire extinguisher administratively labeled number 28, was not mounted. This condition exposed employees to a fire hazard.

29 CFR 1910.1200(f)(5): Chemical manufacturers, importers, or distributors did not ensure that each container of hazardous chemicals leaving the workplace was labeled, tagged, or marked in accordance with 29 CFR 1910.1200. Office area; on or about 21 October 2014 and times prior thereto; an unmarked plastic spray bottle; containing an unknown chemical substance was not marked identifying the product and the hazard warnings associated with it. This condition exposed employees to chemical burns, poisoning, and skin dermatitis.
