29 CFR 1903.19(d)(1): The employer did not submit to the Agency documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.  a) the employer failed to provide documentation and certification for abatement of citation 1 item 1 in which each employee engaged in activities 6 feet or above was not protected by guardrail systems, safety net system, or personal fall arrest system, nor was alternative fall protection measure provided. Condition noted on or about 10/28/14.  b) the employer failed to provide documentation and certification for abatement of citation 1 item 2 in which the employer did not provide a training program for each employees potentially exposed to fall  hazards. Condition noted on or about 10/28/14.   NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a- 6 East 44rth Street, Brant Beach NJ: Employees was exposed to fall hazards, in excess of 22 feet 7 inches to the ground surface below, when performing sheathing installation and other related activities. These employees had not received fall protection training from the company.  Condition noted on or about 10/28/14.   NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATMENT DOCUMENTATION FOR THIS ITEM.
