29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed a- Back of Building, 3 North Beach Boulevard, Surf City: Company workers weres exposed to to fall hazards in excess of 12 feet to the ground surface below when using a portable extension ladder whose side rails did not extend at least 3 feet above the upper landing surface being accessed. Condition observed on or about 10/17/17. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.1053(b)(6): Unsecured ladder(s) were used on surfaces that were not stable and level: a- Back of Building, 3 North Beach Boulevard, Surf City: Company workers were exposed to to fall hazards in excess of 12 feet to the ground surface below when using a portable extension ladder that was not set on a stable and level surface. Condition observed on or about 10/17/17. William R. Sieber Building Contractor, LLC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(6), which was contained in OSHA inspection number 942848, citation number 1, item number 1 and was affirmed as a final order on 01/14/14, with respect to a workplace located at 11310 Sunset Terrace, Long Beach Township NJ 08008. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
