The maximum permissible spans for 2- X 9-inch or wider planks are shown in the following table:    (Table Omitted)    The maximum permissible span for 1 1/4 x 9-inch or wider plank of full thickness is 4 feet with medium loading of 50 p.s.f.    (a) Shop 2 - Employees were exposed to fall hazards of up to 17 feet while walking on plywood boards that were not scaffold grade or intended for the use.     (b) Interior - Employees were exposed to fall hazards in excess of four feet while walking and standing on plywood boards that were not scaffold grade or intended for the use.     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

(a) Quality Pit, DMUs: Employees were exposed to fall hazards up to 17 feet when walking and standing on plywood boards that were temporarily placed on top of the railcar's roof that did not overlap a minimum of 12 inches on each side.    (b) Interior: Employees were exposed to fall hazards in excess of four feet when walking and standing on plywood boards that were temporarily placed in the railcar's interior that did not overlap a minimum of 12 inches on each side and were not secured from movement.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:    Quality Pit, Testing Bay - Employees were exposed to hazardous arcing when handling the Kikusui unit's power supply and AC power cord that was not equipped with an effective strain relief component to prevent strain from being transmitted to the terminal screws in the cord's receptacle.     There is no abatement certification or documentation required for this item.

29 CFR 1910.333(a): Safety-related work practices shall be employed to prevent electric shock or other injuries resulting from either direct or indirect electrical contacts, when work is performed near or on equipment or circuits which are or may be energized. The specific safety-related work practices shall be consistent with the nature and extent of the associated electrical hazards.    Quality Pit Test Bay: Employees were assigned to perform tasks on or near energized electrical parts operating at up to 1500 volts where the nature and extent was not determined prior to selection of safety-related work practices. Employees were exposed to Arc Flash and direct and indirect contact hazards.     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
