29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:    Shop 2, DMU: Employees were positioning a standard guard rail lowered with an overhead crane at the end of the fixed elevated work platform. Employees were exposed to fall hazards up to 12 feet when standing at the open-sided edge of the work platform without fall protection provided.     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).    Note: Toeboards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    Shop 1 Sidewall, Genesis custom spot welder, spot welding gun - hazardous energy control procedures (i.e. lockout tagout) were not utilized for an orderly shutdown of the equipment and isolation of hazardous energy during servicing activity to replace electrodes and other components. Employees assigned to perform this servicing activity had their finger(s) and hand(s) exposed to caught in-between hazards capable of causing serious laceration injury. Use of the two hand trigger controls for actuation did not provide effective protection against unexpected start-up.     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):    Shop 1 Sidewall, Genesis custom spot welder, spot welding gun - prior to starting work, employees replacing (servicing) electrodes and other components in the point-of-operation and associated danger zone were not required to apply energy controls in the following sequence:     (a) turn off and shut down the machines,  (b) physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the energy source,  (c) affix lockout or tagout devices to each energy isolating device,  (d) verify that isolation and deenergization of the machine or equipment have been accomplished.    Employees were exposed to caught-in hazards likely to result in serious laceration injury of hand(s) and finger(s).    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
