29 CFR 1910.134(f)(2): Employee(s) using a tight-fitting facepiece respirator were not annually fit tested: a) Panel Built, Inc., Blairsville, GA - Employees were required to use a half mask respirator and full face respirator in the panel fabrication building to spray foam adhesive containing isocyanates and had not been annually fit tested, on or about 10/29/14. b) Panel Built, Inc., Blairsville, GA - Employees were required to use a half mask respirator in the panel fabrication building to cut gyp/gyp boards containing silica and had not been annually fit tested, on or about 10/29/14. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace: Panel Built, Inc., Blairsville, GA - An employee was issued and used a half mask respirator while assisting in the booth to spray foam adhesive containing isocyanates and had not been medially evaluated on or about 10/29/14. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
