29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings [or equivalent as specified in 29 CFR 1910.23(c)(3)(i) through (v)], on all open sides: Employees were exposed to fall hazards of approximately 23ft when accessing the area on the roof above the dump pit to perform work on the equipment in that area because the employer did not ensure the work area was guarded by a standard railing or equivalent method.
