29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about January 10, 2023, employees were exposed to struck by hazards associated with moving machine parts during servicing and/or maintenance activities performed inside the work envelope of the Fuji Robotics Palletizer Model EC-171. The employer had not ensured control of hazardous energy procedures were utilized to protect employees performing servicing and/or maintenance work inside the work envelope of the Fuji Robotics Palletizer Model EC-171.

29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: On or about January 10, 2023, employees were exposed to struck by hazards associated with moving machine parts during servicing and/or maintenance activities performed inside the work envelope of the Fuji Robotics Palletizer Model EC-171. The employer failed to implement energy control application steps as the lockout device was not affixed to the energy isolating device [per the 1910.147(d)(4)(i) requirement]. As a result, the remaining applicable energy control elements, involving machine dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards.
