29 CFR 1910.28(b)(3)(ii): The employer did not ensure that each employee was protected from tripping into or stepping into or through any hole that is less than 4 feet (1.2 m) above a lower level by covers or guardrail systems: On or about November 4, 2020, an employee was exposed to injuries such as, but not limited to, fractured bones when working on the catwalk adjacent to the north/south drag line conveyor feeding bins 1, 2, and 19. The employer did not ensure the hole between the structural beam and the drag line conveyor housing was protected by a cover or guardrail system.
