29 CFR 1960.8(c): The Agency did not develop, implement, and evaluate an occupational safety and health program in accordance with the requirements of section 19 of the Act, Executive Order 12196, and the basic program elements prescribed in this part, or approved alternate program elements: BOP Medical Center: The employer did not develop and implement a standard operating procedure for TB isolation rooms filters that were not being changed in accordance with manufacturer instructions. ABATEMENT DOCUMENTATION REQUIRED

29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toe boards: BOP FMC, FCI I, FCI II and FCI Low Institutions: Correctional Officers performing roof inspections on buildings up to Five stories high were exposed to unprotected falls of up to 45 feet. Notice Citation 01 Item 002, 29 CFR 1910.23(c)(1) as required by 29 CFR 1960.8(b): The employer failed to ensure employees were protected from a 10 foot fall hazard. The employer was cited for a violation of the same standard on 05/07/2013, Inspection #898432, 06/03/2013 of final order, and was abated by installing standard guard railings around platform used by employees to clean boiler. Notice Citation 01 Item 001, 29 CFR 1910.23(c)(1) as required by 29 CFR 1960.8(b): The employer failed to ensure employees were protected from a 9 foot fall hazard. The employer was cited for a violation of the same standard on 09/03/2013, Inspection #913211, 10/16/2013 of final order, and was abated by tagging out of service until a cage could be built. ABATEMENT DOCUMENTATION REQUIRED
