29 CFR 1910.178(q)(7): Industrial trucks were not examined before being placed in service, and shall not be placed in service if the examination shows any condition adversely affecting the safety of the vehicle. Such examination shall be made at least daily. BOP FCC Butner, NC: The employer did not ensure that each Taylor Dunn or EZGO electrical cart vehicle was examined daily prior to being placed into service in order to check for any adverse condition that could affect the safety of the vehicle, such as but not limited to the interlock underneath the seat, tires and brakes. Notice Citation 01 Item 002, 29 CFR 1910.178(q)(7) as required by 29 CFR 1960.8(b): The employer failed to ensure daily checked were being conducted by all employees authorized to drive such equipment. The employer was cited for a violation of the same standard on 02/04/2014, Inspection #950209, 03/03/2014 of final order, and was abated by updating the Powered Industrial Truck policy to include EZGO's on their checklist. Notice Citation 01 Item 003, 29 CFR 1910.178(q)(7) as required by 29 CFR 1960.8(b): The employer failed to ensure the use of pre-use checklist prior to placing PITs into service. The employer was cited for a violation of the same standard on 02/19/2014, Inspection #947472, 03/17/2014 of final order, and was abated by requiring forklift operators to document daily checks onto institution's daily forklift checklist. ABATEMENT DOCUMENTATION REQUIRED

Notice Citation 01 Item 003, 29 CFR 1960.59(a) as required by 29 CFR 1960.8(b): The employer failed to ensure that each and every employee who operates the EZGO carts was trained. The employer was cited for a violation of the same standard on 02/04/2014, Inspection #950209, 03/03/2014 of final order, and was abated by conducting multiple PowerPoint training sessions to employees. ABATEMENT DOCUMENTATION REQUIRED
