29 CFR 1910.147(c)(1):  Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:       (a)  There was no program on energy control procedures, employee training, and periodic inspections, to include providing locks and training to employees changing blades on the saws and setting up, cleaning and replacing parts on the printers.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.    (a)  On or about November 6, 2014, the Vutek Printer #1 was being run with the guard up and the interlocks bypassed, thereby exposing employees to ingoing nip points and the moving carriage.

29 CFR 1910.213(c)(1): Each circular hand-fed ripsaw shall be guarded by a hood which shall completely enclose that portion of the saw above the table and that portion of the saw above the material being cut. The hood and mounting shall be arranged so that the hood will automatically adjust itself to the thickness of and remain in contact with the material being cut but it shall not offer any considerable resistance to insertion of material to saw or to passage of the material being sawed. The hood shall be made of adequate strength to resist blows and strains incidental to reasonable operation, adjusting, and handling, and shall be so designed as to protect the operator from flying splinters and broken saw teeth. It shall be made of material that is soft enough so that it will be unlikely to cause tooth breakage. The hood shall be so mounted as to insure that its operation will be positive, reliable, and in true alignment with the saw; and the mounting shall be adequate in strength to resist any reasonable side thrust or other force tending to throw it out of line:    (a)  On or about September 22, 2014, the Powermatic Table Saw, Model #66, S/N 876672184, was unguarded, thereby exposing the operator to the revolving blade.

29 CFR 1910.1200(h)(3)(iv): The details of the hazard communication program developed by the employer, including an explanation of the labels received on shipped containers and the workplace labeling system used by their employer; the safety data sheet, including the order of information and how employees can obtain and use the appropriate hazard information:  (a) The employer failed to provide adequate training on the hazard communication program, including the location of the material safety data sheets (MSDS) and how employees could obtain appropriate hazard information from the material safety data sheets on the solvents, lacquer thinner and denatured alcohol that is used in the shop.
