29 CFR 1926.503(a)(2): The employer did not assure that each employee exposed to fall hazards was trained by a competent person qualified in the areas specified in 29 CFR 1926.503 (a)(2)(i) through (viii)  The employer is failing to protect employees from fall hazards while engaging in roofing work in that; adequate fall protection training was not provided by a competent person to employees  working at a height of 22' eave to ground while installing roofing membrane material on a 2 story house with a 6 in 12 pitch located at 2469 182nd Cir, Omaha, NE 68130.  Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citations.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):  The employer is failing to protect employees from fall hazards while engaging in roofing work in that; fall protection was not utilized by employees  working at a height of 22' eave to ground while installing roofing membrane material on a 2 story house with a 6 in 12 pitch located at 2469 182nd Cir, Omaha, NE 68130.  Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citations.  The Francisco Esquivel was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 314057225, citation number 1, item number 1 and was affirmed as a final order on 11/18/2010, with respect to a workplace located at 5401 N 155th St, Omaha, NE 68116.
