On or about November 10, 2014, at the south side of the roof, three employees operating pneumatic nail guns for framing work were not wearing eye protection, being exposed to get struck-by flying debris.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a. On or about November 10, 2014, two employees sheathing a roof frame of 7:12 pitch were exposed to a 10 ft. 8 in. fall hazard without the use of personal fall arrest system, safety net system or guardrail system. b. On or about November 10, 2014, an employee installing a fascia on a 7:12 pitch roof frame was exposed to a 17 ft. 6 in. fall hazard without the use of personal fall arrest system, safety net system or guardrail system.
