29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s): a) On or about October 2, 2014, Mountain State Home Improvement, as a controlling employer, did not provide or maintain programs and conduct inspections of the job site to ensure that Montez Roofing (subcontractor) protected their employees from falls at approximately 23.3 feet while performing re-roofing activities. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet"). Mountain States Home Improvement, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 314657867, citation number 1, item number 1 and was affirmed as a final order on July 27, 2010, with respect to a workplace located at 10517 Vaughn St., Commerce City , Colorado 80013. The final abatement date for this citation was August 20, 2010.
