29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s): (a) On or about June 11, 2019, Mountain States Home Improvement, Inc., as the controlling employer, did not conduct frequent and regular inspections of the job-site to ensure that Yasser Contractors, LLC (subcontractor) protected their employees from falls while performing re-roofing activities. Abatement Note: Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet"). Mountain States Home Improvement, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number, 1007537 citation number 1, item number 1 and was affirmed as a final order on December 30, 2014, with respect to a workplace located at 19548 E. Powers Pl. , Aurora, CO 80015. Mountain States Home Improvement, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number, 988428, citation number 1, item number 1 and was affirmed as a final order on November 7, 2014, with respect to a workplace located at 20532 Caley Lane, Aurora, CO 80015.
