OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, including blunt force trauma, in that employees were exposed to being struck-by a radial fan impeller blade: b) Crystallizer Building, Blower Room: On 11/28/2014 at 04:30 PM, a catastrophic failure of blower B20 caused its 4'10" stainless steel impeller to break through its housing and parts of the impeller broke through the building's roof and a wall. Prior to the failure of blower B20, the stainless steel impellers on the other two radial fans in the series, B21 and B22, developed fractures, were taken out of service, and replaced with high-strength carbon steel impellers. The employer continued to run the blower B20, ramped the blower up to 99% of its maximum speed of 3300 RPM, and did not inspect the B20 impeller for weaknesses and fractures, via appropriate qualitative and quantitative analysis, to ensure safe operation of the radial fan, discovered on or about 12/02/2014. ABATEMENT CERTIFICATION REQUIRED
