29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) In the blending rooms- employees were exposed to struck by hazards when working around the O'Hara V-Shell Tumble Blenders without guards preventing workers from entering the rotating parts of the blenders. This violation was observed on or about November 17, 2014.

29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries: a) In the hallway on the way to the blending rooms, employees were exposed to fire hazards when a fire extinguisher was missing from where signs or placards claimed that it should be located. This violation was observed on or about November 17, 2014.
