29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: Note: As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated On or about November 11, 2014 and times prior thereto, at the workplace located at or near 8603 Pinnacle Valley Rd. Little Rock, Arkansas, an employee operated an aerial lift without attaching the lanyard onto the anchor point in the basket 8 feet above the ground. This condition exposed the employee to the hazard of falls from elevation.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and safety monitoring system. C.B.M. Construction Co., Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(10), which was contained in OSHA inspection number 899321, citation number 1, item number 1 and was affirmed as a final order on 05/08/2013, with respect to a workplace located at 5125 Northshore Drive in North Little Rock, Arkansas. On or about November 17, 2014 and times prior thereto, at the workplace located at or near 8603 Pinnacle Valley Rd. Little Rock, Arkansas, employees were working on a roof 12 feet above the ground without fall the use a fall protection system. This condition exposed employees to the hazard of falls from an elevation.
