29 CFR 1910.215(b)(3): Bench and/or floor stand grinding machinery had angular exposure of the grinding wheel periphery and sides for safety guard(s) which exceeded 125 degrees where the work required contact with the wheel below the horizontal plane: a) On or about November 19, 2014, the employer did not assure the angle exposure of the grinding wheel periphery on the pedestal grinder in the ECI cell in the finishing department was adequate. The exposure angle was too large exposing employees to caught-in and struck by hazards when operating the grinder.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: a) On or about November 19, 2014, the employer did not assure that both sides of the pedestal grinder in the ECI cell in the finishing department was equipped with a tongue guard positioned within 1/4 inch of the abrasive wheel's peripheral edge. Employees that operated the grinder to debur parts were exposed to struck-by injuries associated breakage of the wheel.
