29 CFR 1910.178(o)(1): Load(s) on powered industrial truck(s) were not stable or safely arranged:    a. Ross Aluminum Castings, LLC - Sidney, OH - On or about February 29, 2016, the employer did not ensure that molds in the assembly area were loaded and moved in a safe and secure manner to prevent the load from becoming removed from the forks.  Large molds were manually pushed onto a loading plate that was not stable/secured to the forklift, this exposed employees in the area to struck-by and crushing injuries.
