Located in the sawmill building at 2400 Larson Road in Gnadenhutten, Ohio, employees who operated the HMC Double End Trim Saw were exposed to unguarded conveyor belts adjacent to the operator's work platform, exposing employees to a struck-by hazard. b. Located in the sawmill building at 2400 Larson Road in Gnadenhutten, Ohio, employees who operated the Meadows Edger were exposed to a fall of approximately 3 foot onto a belt conveyor used to transport sawdust and trimmings.

29 CFR 1910.147(c)(1): Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative. a. Located at 2400 Larson Road in Gnadenhutten, Ohio, the employer did not have an adequate energy control program that included lockout and tagout procedures, training, and periodic inspections, thereby exposing employees to amputation and electrocution hazards. Lockout/Tagout was needed to remove all energy sources from the mill equipment, including but not limited to mill saws, conveyors, an edger, and a chipper.

The unguarded belt was approximately 6 inches from the floor.

Located in the sawmill building at 2400 Larson Road in Gnadenhutten, Ohio, employees were exposed to unguarded chain and sprocket while operating the edger machine, exposing employees to an amputation hazard. c. Located in the sawmill building at 2400 Larson Road in Gnadenhutten, Ohio, employees were stacking lumber beside a HMC Double End Trim Saw with an unguarded spiked chain, exposing employees to a caught-in hazard.

Sufficient access and working space shall be provided and maintained about all electric equipment to permit ready and safe operation and maintenance of such equipment. a. Located in the sawmill building at 2400 Larson Road in Gnadenhutten, Ohio, an electrical disconnect was mounted inside the frame of the chain conveyor at the outlet side of the trim saw. The disconnect was not easily accessible for employees to service.
