At the work site located at 2400 Larson Road SE, the employer failed to ensure that all elevated surfaces such as, but not limited to, pipes, ledges, building support members and equipment, were kept free of hazardous accumulations of combustible dust, exposing employees to fire, deflagration and dust explosion hazards.

At the work site, the employer had not developed and/or implemented a monitoring system for employees who worked in the mill and were exposed to noise at an average (TWA) sound level of 85 decibels or greater as measured on the A scale (slow response). Three employees who worked in the mill were exposed to noise at an average (TWA) sound level of 92 decibels or greater as measured on the A scale (slow response) during sampling on December 23, 2014.

29 CFR 1910.95(g)(1): The employer shall establish and maintain an audiometric testing program as provided in this paragraph by making audiometric testing available to all employees whose exposures equal or exceed an 8-hour time-weighted average of 85 decibels. a. At the work site, the employer had not provided a baseline audiogram for employees who worked in the mill and were exposed to noise at an average (TWA) sound level of 85 decibels or greater as measured on the A scale (slow response). Three employees who worked in the mill were exposed to noise at an average (TWA) sound level of 92 decibels or greater as measured on the A scale (slow response) during sampling on December 23, 2014. b. At the work site, the employer had not provided annual audiometric testing for employees who worked in the mill and were exposed to noise at an average (TWA) sound level of 85 decibels or greater as measured on the A scale (slow response). Three employees who worked in the mill were exposed to noise at an average (TWA) sound level of 92 decibels or greater as measured on the A scale (slow response) during sampling on December 23, 2014.

You must enter information about your business at the top of the OSHA 300 Log, enter a one or two line description for each recordable injury or illness, and summarize this information on the OSHA 300-A at the end of the year. a. At the facility located at 2400 Larson Road, Gnadenhutten, Ohio, at the time of inspection, the employer had not recorded information in column (F) correctly for calendar year 2014 a description of work-related injuries and illnesses. On or about November 4, 2014, an employee had a fractured finger but there was no description of what caused the injury, which specific finger was injured, and/or what the employee was doing when the injury occurred.
