29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) For employees working at 11210 Southwind Lake Drive in Gibsonton, as observed on or about November 24, 2014. Employees were observed installing plywood sheathing on the roof with a 5:12 pitch. The eave to ground height measured approximately 9 feet 11 inches and the employees had no fall protection system in place.   b) For employees working at 11210 Southwind Lake Drive in Gibsonton, as observed on or about November 24, 2014. Employees were observed installing plywood sheathing on the roof with a 5:12 pitch. The employees were stepping across an approximately 8 inch opening on the roof to access the ladder. The eave to ground height measured approximately 9 feet 11 inches.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) For employees working at 11210 Southwind Lake Drive in Gibsonton, as observed on or about November 24, 2014. Employees were observed installing plywood sheathing on the roof with a 5:12 pitch. The employees were accessing the roof with a portable ladder that was not extended a minimum of 3 feet above the upper landing surface. The eave to ground height measured approximately 9 feet 11 inches.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:  a) For employees working at 11210 Southwind Lake Drive in Gibsonton, as observed on or about November 24, 2014. Employees were observed installing plywood sheathing on the roof with a 5:12 pitch. The employees were using a stepladder that was in the closed position leaning up against the eave to gain access to the upper working surface. Ladder is designed to be used in the open position with the spreader bars locked.

29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):  a) For employees working at 11210 Southwind Lake Drive in Gibsonton, as observed on or about November 24, 2014. Employees were observed using a ground fault circuit interrupter that was defective. Ground fault circuit protector was physically damaged and inoperable and no inspection prior to use was performed.
