29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  a.   On or about Dec. 2, 2014, on the northeast side of the house, an employee working on a jobsite cutting wood while using a circular saw did not wear eye protection, exposing the employee to the hazard of being struck in the eye or face by flying debris.     b.  On or about Dec. 2, 2014, on the northeast side of the house, an employee working on a jobsite using a pneumatic nail gun did not wear eye protection, exposing the employee to the hazard of being struck in the eye or face by flying debris.     Signature USA Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 981785, citation number 01, item number 02 and was affirmed as a final order on August 13, 2014, with respect to a workplace located at Medley Street, Lot 73, Panama City Beach, FL 32413.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a.   On or about December 2, 2014, on the south side of the house, the ladder used to access the roof did not extend 3 feet above the upper landing surface, exposing employees to an 12 feet 3 inch fall hazard.  Signature USA Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 943852, citation number 01, item number 03 and was affirmed as a final order on November 16, 2013, with respect to a workplace located at 12983 US Highway 98, Panama City Beach, FL 32407.
