29 CFR 1910.132(d)(1): The employer shall assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE). Location - 6 Rowe Square, Gloucester, MA On or about 12/1/14, an employee was operating truck #400, Mack MR600 winch control cable and the employee's hand was caught and crushed between the handle that raises the sweep blade and the steel winch cable, amputating the left index finger to the first knuckle and lacerated the left ring finger requiring stitches and has not returned to work.
