29 CFR 1910.119(f)(1): The employer did not develop and implement written operating procedures that provide clear instructions for safely conducting activities involved in each covered process consistent with the process safety information: (a) On or about 10/2/14 - at Building A4, the procedure PW04 "Rework Process for Priming Comets" dated 9/18/14 did not provide clear instructions that were consistent with the process safety information contained in the material safety data sheets for Potassium perchlorate, Nitrocellulose Wet with Water, and Toluene that state precautions such as but not limited to: do not store in direct sunlight or expose to UV radiation; keep away from heat. (b) On or about 10/2/14 - at Building A4, the Static Electricity Program did not provide clear instructions to maintain the bonding and grounding of mixing bowls or other equipment that contain flammables, such as but not limited to, Toluene.
