29 CFR 1926.602(c)(1)(ii): No modifications or additions which affect the capacity or safe operation of the equipment shall be made without the manufacturer's written approval. If such modifications or changes are made, the capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly. In no case shall the original safety factor of the equipment be reduced. a. At the construction site located at 1835 Finland Ave., Columbus, Ohio, a Genie rough terrain forklift, model number GTH-1056 was used to hoist steel bar joists by placing a sling onto the tines of the forklift for a below-the-tines lift (free rigging), a modification, thereby exposing the employees to a struck-by hazard.

You must enter information about your business at the top of the OSHA 300 Log, enter a one or two line description for each recordable injury or illness, and summarize this information on the OSHA 300-A at the end of the year. Located at The Construction Crew, LLC. in Logan, Ohio: a. On the 2014 OSHA 300 Log the employer did not provide all information required in Column F such as the actual cause of injury. 1. For the injury that occurred on August 18, 2014, column F stated, "crushed elbow and forearm", which is insufficient to determine which elbow and forearm was injured and what caused the injury.
