29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section:  a.   The employer fails to assure effective protection against falls as required by the standard, as demonstrated on December 15, 2014, when employees installing roofing materials on the 6:12 pitch roof at 9902 Blakeford Mill Road, Jacksonville, Florida, 32256 were exposed to an 8 feet to 17 feet fall hazard from the roof eaves in that they were not protected by a fall protection system.  Carlson Enterprises, LLC. previously committed a Repeat violation of this standard on August 13, 2013, when employees were working on a residential house at 2588 Spring Meadows Drive, Middleburg, Florida, 32050, which became a Final Order on September 16, 2013. (Inspection No. 929511)  Carlson Enterprises, LLC. previously committed a Serious violation of this standard on October 15, 2012, when employees were working on a residential house at 181 Nadia Michelle Drive, Jacksonville, Florida, 32225, which became a Final Order on November 20, 2012. (Inspection No. 687898)  Carlson Enterprises, LLC. previously committed a Serious violation of this standard on May 24, 2012, when employees were working on a residential house at 12551 Glamdring Court, Jacksonville, Florida, 32225, which became a Final Order on June 11, 2012. (Inspection No. 446956)  To abate this violation, the employer must ensure that all employees wear appropriate fall protection or install a guardrail or safety net system when an employee works above 6 feet off the surface.  Pursuant to 29 CFR 1903.19, the employer must submit documents describing the steps it is taking to ensure compliance, including and explanation of how these steps protect its employees from fall hazards.
