The metal knockout box was hanging from a ceiling beam and being used in conjunction with the horizontal band saw's power cord.

Midtown Pallet & Recycling, Inc. - Defiance, Ohio: On or about December 10, 2014, the employer did not ensure that the flexible cord, used to power the table saw, was connected to fittings equipped with strain relief.  The flexible cord was pulled from the plug end of the cord located at the wall outlet.   b. Midtown Pallet & Recycling, Inc. - Defiance, Ohio: On or about December 15, 2015, the employer did not ensure that the flexible cord from the motor of the horizontal band saw (pallet dismantle saw) was connected to fittings equipped with strain relief.  The flexible cord which powers the saw from the saws motor was pulled from fittings.

Midtown Pallet & Recycling, Inc. - Defiance, Ohio: On December 17, 2014, the employer did not ensure the belt and pulleys, which transfer power from the trim track saw motor to the saw, were guarded.  Employees were exposed to hazards create by the rotating parts and nip- points.    c. Midtown Pallet & Recycling, Inc. - Defiance, Ohio: On or about December 17, 2014, employees were exposed to in-running nip-points and rotating parts hazards of the trim track saw.  The employer did not ensure that ingoing nip-points of the conveyor track, and of the gears and chain and sprocket used to transfer power from the conveyor motor to the conveyor track, were guarded to protect employees from injury.
