29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: (a) 22 Reading Blvd Wyomissing PA - Employee(s) were observed performing roofing work activity on a 4 in 12 pitch roof without the benefit of a fall protection system, thereby exposing employees to a fall of approximately 9 feet 4 inches to the ground below, on or about December 16, 2014. No abatement certification or documentation required.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: (a) 22 Reading Blvd Wyomissing, PA - Employees used one of three (3) aluminum extension ladders to access the roof of a residential structure. The side rails of the ladders did not extend at least 3 feet above the upper landing surface thereby exposing these employees to a fall hazard of approximately 9 feet 4 inches to the ground below, on or about December 16, 2014. No abatement certification or documentation required. SM Contractors LLC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.1053(b)(1), which was contained in OSHA inspection number 909031, citation number 1, item number 1a, and was affirmed as a final order on October 29, 2013, with respect to a workplace located at 331 Clayton Manor Drive Middletown, DE.
