29 CFR 1926.502(d)(17): The attachment point of the body belt was not located in the center of the wearer's back. The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head. a) Doveview Drive-Roof - On or about January 25, 2017, the employer failed to ensure the employees attached their lanyards to the center of the employee's back, in that employees wearing a full body harness connected their lanyard to the hip D-rings designed to be used for positioning, exposing employees ton a fall hazard of approximately 37 feet.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: a) Doveview Dr. Dover, DE-On or about January 25, 2017, the employer failed to ensure that employees engaged in roofing work at heights of approximately 37 feet to grade received training related to fall hazards and the procedures to be followed in order to minimize these hazards. THE EMPLOYER SM CONTRACTORS, LLC WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 1926.503(A)(1), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 909031, CITATION NUMBER 2, ITEM NUMBER 2 AND WAS AFFIRMED AS A FINAL ORDER ON OCTOBER 29, 2013, WITH RESPECT TO A WORKPLACE LOCATED AT CLAYTON MANOR DRIVE, MIDDLETOWN, DELAWARE.
