Moodys Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 315479841, citation number 01, item number 001 and was affirmed as a final order on September 22, 2011, with respect to a workplace located at 11137 Normandy Blvd., Jacksonville, FL.   29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:    a.  On or about December 18, 2014, at the north and south side of the roof, two employees operating pneumatic nail guns to install shingles were not wearing eye protection, being exposed to get struck-by flying debris.

Moodys Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 528260, citation number 01, item number 001 and was affirmed as a final order on August 21, 2012, with respect to a workplace located at 6731 Normandy Blvd., Jacksonville, FL.    29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):      a. On or about December 18, 2014, five employees installing shingles on a 7:12 pitch roof were exposed to an 10 ft. 2 in. fall hazard, and higher, without the use of personal fall arrest system, safety net system or guardrail system.
