(b) Location - Establishment On or about 1/7/15 the employer did not carry out the provisions of the Chemical Hygiene Plan by evaluating the ventilation efficiency of glove boxes, gas cabinets, and other ventilation engineering controls (protective equipment) to ensure proper and adequate performance.
