The inspection procedures utilized at the facility did not establish a set of specific actions and strategies detailing the scope, extent, methods and timing of specific inspection activities in order to determine the condition of a pressure vessel based on defined and/or expected damage. Pressure vessels inspected using inadequate written procedures include at least GL2001, GL2002, and PFN2. This condition exposed employees to the hazards of fire/explosion and toxic release. (b) Location - 9 Opportunity Way, Newburyport, MA: On or about December 4, 2018, adequate written mechanical integrity procedures for process hose inspections have not been developed. The inspection procedures utilized at the facility did not establish a set of specific actions and strategies detailing the scope, extent, methods and timing of specific inspection activities in order to determine the condition of a process hose based on defined and/or expected damage. Process hoses inspected using inadequate written procedures include at least CH-192, CH-209, CH-211, CH-212, CH-213, CH-216, CH-221, CH-222, CH-227, CH-229, CH-231, CH-232, CH-233, CH-234, CH-235, CH-236, CH-237, CH-238, CH-247, CH-249, CH250, CH-251, CH-270, CH-280, CH-281, CH-282, CH_283, and CH-284.
