29 CFR 1910.132(d)(1)(i): The employer did not select and have each affected employee use, the types of personal protective equipment that would protect the affected employee(s) from the hazards identified in the hazard assessment: (a) North and South Foundry: Employees working around the foundry furnaces were exposed to burns to the skin as there was not a hazard assessment specifying the required personal protective equipment when working with molten aluminum. Gloves and other personal protective equipment should be selected based on working with the molten aluminum which is approximately 1100 degrees Fahrenheit.

29 CFR 1910.138(a): The employer did not select and require employee(s) to use appropriate hand protection when employees' hands were exposed to hazards such as those from skin absorption of harmful substances; severe cuts or lacerations; severe abrasion; punctures; chemical burns; thermal burns; and harmful temperature extremes. (a) North Foundry: Employees were exposed to third degree burns of the skin as appropriate hand protection was not worn when employees were taking metal samples from the furnaces. Gloves available to employees were rated up to 400 degrees, when aluminum furnace is approximately 1100 degrees Fahrenheit. On or about January 4, 2015, an employee received third degree burns when appropriate hand protection for working with hot metal was not being required when taking metal samples from the foundry in Cell 13 and Cell 14.

(b) North Foundry: Employees were exposed to electrical hazards as the Tannewitz Power Series Vertical Band Saw did not have adequate strain relief as the outer insulation had pulled back exposing the interior wiring to the Allen Bradley on/off switch. (c) North Foundry, Cell S9 : Employees were exposed to electrical hazards as the MBD Vertical Band Saw did not have adequate strain relief as the outer insulation had pulled back exposing the interior wiring.
