The employee was exposed to amputation hazards and at risk of serious injury.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) Cooper Farms Inc., #1 Cooper Farms Drive, St. Henry, Ohio: On or about January 7, 2015, the employer did not utilize one or more methods to guard the rotating star wheel on Leg Processor 3 and 4. Employees were at risk of serious injury from the rotating wheel. (b) Cooper Farms Inc., #1 Cooper Farms Drive, St. Henry, Ohio: On or about January 7, 2015, the employer did not utilize one or more methods to guard the ingoing nip points created by the moving chain conveyor on Leg Processor 3 and 4. (c) Cooper Farms Inc., #1 Cooper Farms Drive, St. Henry, Ohio: On or about January 7, 2015, the employer did not utilize one or more methods to guard the ingoing nip points created by the rotating screw conveyors on Leg Processor 3 and 4.

29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven -7 feet or less above floors or platforms were not enclosed: (a) Cooper Farms Inc., #1 Cooper Farms Drive, St. Henry, Ohio: On or about January 7, 2015, the employer did not ensure that the sprocket and "sticker" chain of the Leg Processors 3 and 4 were enclosed. Employees working on the catwalk between the two processors were at risk of serious injury from the rotating sprocket and chain. (b) Cooper Farms Inc., #1 Cooper Farms Drive, St. Henry, Ohio: On or about January 7, 2015, the employer did not ensure that the gears, sprockets, and drive chains for the star wheel on Leg Processor 3 and 4 were enclosed to prevent access. The moveable barrier guard covering the drive mechanism was not interlocked or fastened closed. Employees working on the catwalk between the two processors were at risk of serious injury.
