29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section. a. Cooper Farms, Inc. located in Van Wert, OH: On or about June 7, 2018, the employer did not ensure the machine specific lockout procedures for the browner contained information on how to isolate oil valve #29 leading to the browner from the day tank. If the valve were to be opened during sanitation or maintenance operations it would introduce hot oil into browner, exposing employees to burn hazards.

29 CFR 1910.147(d): The established procedures for the application of energy control (the lockout or tagout procedures) did not cover the following elements and actions and were not done in the following sequence. a. Cooper Farms, Inc. located in Van Wert, OH: On or about June 7, 2018, the employer did not ensure the pump supplying oil to the day tank was locked out by the sanitization crew prior to filling the browner with water. If the pump were to activate during the sanitation process, employees would be exposed to hot oil and steam from material erupting from the day tank as water mixed with hot oil. b. Cooper Farms, Inc. located in Van Wert, OH: On or about June 7, 2018, the employer did not ensure valve #29 was locked out by the sanitization crew prior to filling the browner with water. If the valve were to be opened during the sanitation operation it would introduce hot oil into the water filled browner, exposing employees to hot oil and steam hazards. c. Cooper Farms, Inc. located in Van Wert, OH: On or about June 7, 2018, the employer did not ensure the electric motor which raises the browner lid, was locked out by the sanitization crew prior to starting sanitation operations. If the lid were to lower unexpectedly employees would be exposed to a crushing hazard between the lid and the frame of the machine. Cooper Farms, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29CFR 1910.147(d), which was contained in OSHA inspection number 1093843, citation number 1, item number 1 and was affirmed as a final order on December 23, 2015, with respect to a workplace located at Cooper Farms Drive, Saint Henry, OH.
